Applies the nursing process to plan and manage the care of complex patients in a comprehensive manner. Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the case management triad model. Facilitates appropriate follow up care and/or refers patients (within the designated patient population) to the appropriate next level or site of care. Serves in an advocacy role on behalf of patients, families, and caregivers to ensure safe, equitable healthcare provision. Ensures evidence-based care and continuously monitors patient outcomes. Furthers professional growth by engaging in learning activities according Board of Nursing standards.

Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.

Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.

Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.

Employs strategies to promote health and a safe and healthy environment.

Nursing Patient Assessment & Evaluation (Advanced): Conducts primary care patient interviews and physical examination. Often takes a lead role in complex patient care situations. Possesses expert knowledge, training and experience to mentor less experienced peers.

Nursing Patient Education (Advanced): Consistently delivers patient education services in a creative and imaginative manner while setting a standard for others. Care Planning (Advanced): Leads members of the health care team to identify desired outcomes. Able to adapt to changes to assignments while maintaining a high level of performance. Develops creative ideas and takes innovative action to achieve planned goals and objectives. Possesses expert knowledge, training, and experience to role model and coach less experienced peers.
