divh2Certified Home Health Aide/h2pThe Certified Home Health Aide (CHHA) provides personal care and homemaking services essential to the patients health care and comfort at home under the direction, instruction and supervision of a Registered Nurse, including companionship, errands, laundry, meal planning and preparation, serving meals and assisting the patient with ADLs. Services are provided within the clients home, hospital, and/or long term care facilities./ppEssential Duties and Responsibilities include the following. Other duties may be assigned./pulliFollows the plan of care to provide safe, competent care to the patient./liliHelps the patient to maintain good personal hygiene and assists in maintaining a healthy, safe environment./liliPlans and prepares nutritious meals, markets when instructed to do so by the nurse./liliAssists the patient with ambulation as ordered by the physician and approved and supervised by the nurse./liliAssists with physical therapy exercises./liliEncourages the patient to become as independent as possible according to the nursing care plan/liliAttempts to promote patients mental alertness through involvement in activities of interest/liliGives simple emotional and psychological support to the patient and other members of the household and establishes a relationship with patient and family, which transmit trust and confidentiality./liliImmediately reports any change in the patients mental or physical condition or in-home situation to the staff nurse, or to the Aide supervisor./liliPerforms routine housekeeping tasks as related to a safe and comfortable environment for the patient and as instructed in the care plan./liliAttends company trainings as requested/liliCompletes necessary Continuing Education Credits as scheduled/required./li/ulpQualifications/ppTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must have completed a homemaker home health aide training program approved by the NJ Board of Nursing and shall be certified by the Board and provide evidence of certification. CPR certification preferred. Must have passed Level II background check and drug screening./ppLanguage Skills/ppAbility to communicate effectively with patient/client, family members, clinical management and staff. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence./ppOther Skills and Abilities/ppCommunication skills, light housekeeping skills, cooking, cleaning, and shopping. Good physical and mental health. Caring attitude, tact, patience, and good personal hygiene. Must be free from health problems that may be injurious to patient, self and co-workers and must present appropriate evidence to substantiate this.
