Ambulatory Care Pharmacist - Primary Care - Full Time

Our pharmacist conducts virtual patient visits from the comfort of their own homes. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
The following shift(s) are available:
~8-hour shift (Monday-Friday; We are a leading physician group serving South Carolina and Georgia, dedicated to delivering quality healthcare directly to patients in care facilities, homes, clinics, and virtual visits. Our services include comprehensive primary care, specialty services, and pharmacy support, tailored to meet diverse patient needs. Provide medication therapy management (MTM) services, including comprehensive medication reviews, medication reconciliation, and adherence assessments.
Collaborate with interdisciplinary care teams to develop and implement comprehensive care plans for high-risk patients with complex medical needs.
Collaborate with internal and external resources to facilitate and ensure seamless operations.
Meet staff efficiency expectations by performing consultations, medication reviews, and other clinical pharmacy services efficiently and effectively.
Maintain accurate and up-to-date documentation of patient encounters, medication therapy interventions, and clinical recommendations in electronic health records (EHR) systems.
Generate reports, analyze medication-related data, and participate in quality improvement initiatives to enhance patient care outcomes and medication safety.
Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program, required.
Licensed Pharmacist in good standing with the state board of pharmacy.
Clinical pharmacy experience preferred.
Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
Basic computer knowledge.
Must be able to work flexible hours and travel between offices, facilities, etc.
