Responsibilities Obtain medical history, particularly as it relates to the present condition, from the patient and/or family member.
Conduct a brief physical examination of the patient, including vital signs, ambulatory status, mental status, bowel and urinary regularity, appetite and type of diet.
Interpret nursing services of the agency to patients and families as part of planning of care.
Provide skilled care and maintain confidentiality of client’s situation, records and services provided according to HIPAA regulations.
Evaluate the effectiveness of nursing service to the individual and family.
Submit all patient notes and/or visit records on a timely basis.
Develop, implement, participate in nursing care plans and coordinate total patient care.
Evaluate the patient’s condition and home situation, determine if services of an LPN, Home Health Aide, or Personal Care Attendant will be required and the frequency of this situation.
Obtain orders for paramedical services and submit referral to the Director of Clinical Services (e.g., physical therapy, medical social services).
Coordinate and supervise the care given by the LPN, Home Health Aide and/or Personal Care Attendant assigned to provide care to the patient.
Qualifications Licensed RN.
OASIS and Wellsky (Kinser) experience strongly preferred.
Med-surge and wound care experience preferred.
Physical within 6 months prior to first patient contact.
Current TB results (within last 12 months) and CPR Certification.
Valid driver’s license and proof of liability insurance on automobile.
Ability to attend 1/2/3-day orientation at our Las Vegas office.
