Provides oversight and general nursing care and services to persons with behavioral health challenges by performing the duties listed in this job description. Relies on education, skill, and judgment to provide quality services to the people we serve with limited direction and supervision. Requires an unencumbered Nursing License.
Pay: $31.50/LPN $37.00/RN
Certified Registered Nurse/ LPN

Five years' experience in the human services field working with individuals with behavioral and or mental health issues (preferred)

Monitor healthcare needs of individuals receiving services and coordinate medical, dental, and vision care services and appointments.
Provide assistance to individuals receiving services who are unable to self-administer medication and supervise self-administered medication and injection of medications, as prescribed.
Provide first aid treatment to individuals receiving services, as needed.
Monitor medications and first aid supplies to ensure expiration dates are not exceeded and adequacy of inventories.
Work with direct care staff to assist individuals receiving services in the areas of personal hygiene, self-care, grooming and other daily living areas, as needed.
Maintain contact/communication with primary care physicians, dentists, and other healthcare specialists.
Prepare and complete all necessary documentation, including assessments, nurses notes, medication administration information, and miscellaneous paperwork/reports as requested.
OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
Drug testing
Education verification and other credentialing based on position requirements.
Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
