Under the general supervision of the Supervisor / Manager, Ambulatory Pharmacy, and within established organizational and departmental policies and procedures, the Ambulatory Pharmacist is responsible for providing pharmaceutical care in a community practice setting. Must be willing / able to support the clinical pharmacy and other organizational activities as required. A commitment to patient safety, customer satisfaction, efficient / effective use of resources, teamwork, innovation, and performance improvement is required
Job Purpose: • Checks for safety, accuracy, appropriateness and completeness with respect to drug, dose, route, frequency with consideration given to patient-specific data including laboratory values, age, gender, height, weight and BSA. • Prepares controlled substances to be dispensed, completing the appropriate documentation while maintaining an accurate perpetual inventory. • Performs weekly inventory and reconciliation of controlled substances. Notifies Manager, Ambulatory Pharmacy Services or Senior Director of Pharmacy of any unexplained discrepancies. • Performs daily oversight of all Pharmacy Technician activities; checks/reviews all Pharmacy Technician work according to regulatory requirements. • Dispenses medication to ambulatory patients with appropriate medication counseling. • Effectively interacts with prescribers concerning drug order clarification or drug therapy modification • Ability to perform without direct professional supervision and to oversee the work of Technicians and students.
Education/Training: Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.Licensure: Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. Work Experience: Minimum of three (3) years experience as a pharmacist is preferred.
