Physical Therapist - Therapy - Part-Time

Advanced Therapy Services PLLC is dedicated to advancing physical and cognitive rehabilitation so patients can achieve meaningful, lasting outcomes. We provide care both in the comfort of patients’ homes and in our outpatient therapy clinic, using outcome-driven treatment protocols. Our experienced therapists and healthcare professionals prioritize personalized care tailored to each individual’s unique needs. This is a part-time, on-site Physical Therapist role based in Crystal Lake, IL. The Physical Therapist will evaluate patients’ physical function, mobility, and pain levels, and develop individualized treatment plans that support rehabilitation goals. The role involves close collaboration with other healthcare professionals, regular documentation of progress notes and treatment outcomes, and timely communication with patients, caregivers, and referring providers. The Physical Therapist will contribute to a supportive clinic environment that promotes safety, respect, and high-quality patient care.

Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and adjust individualized treatment plans.

Demonstrated skills in Patient Care and Home Care, including patient and caregiver education and safe progression of exercises.

Working knowledge of Medicine principles relevant to musculoskeletal, neurological, and functional conditions treated in outpatient settings.

Current state licensure as a Physical Therapist (or eligibility to obtain licensure in Illinois).

Doctor of Physical Therapy (DPT) or equivalent accredited degree.

Experience in outpatient or home health rehabilitation is preferred.

Ability to perform safe patient handling, including transfers and mobility assistance as needed.
