ACT II - Registered Nurse (Full Time)

Assess, monitor and address the psychiatric, mental health, medical and care coordination needs of clients enrolled in Community Support Services Integrated Health Home.

EDUCATION AND TRAINING REQUIRED:

RN required, Bachelor's degree preferred. Experience and/or interest in working with individuals with chronic psychiatric impairment and/or chronic medical conditions and/or substance addictions
Must be able to read, write, speak and comprehend English.
Must possess or be willing to develop basic computer skills and utilize an electronic health record
Must be willing and physically able to outreach clients in multiple community and residential settings, including via stairs, and including in inclement weather, and including in occasional incidence of residence infestation (personal protective gear and education is provided by employer)
Must be willing to interact with individuals diagnosed with infectious diseases, adhering to all agency infection control guidelines and recommendations
