This position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties. Under general supervision, serves as Linkage Coordinator for HIV Services and is responsible for navigating activities related to HIV testing, linkage to HIV medical care, retention and the re-engagement of persons living with HIV into medical care, treatment, and support services. Perform HIV testing, counseling, and outreach for high-risk residents in the community. Ensure implementation of timely disease intervention strategies to decrease the prevalence of HIV and STDs. This is a grant-funded position under the HIV Prevention Program.

Implement the Antiretroviral Treatment and Access to Services (ARTAS) strategy for individuals newly diagnosed with HIV or lost to care

Coordinate timely linkage, referrals, and follow-up for HIV care, partner services, STI and hepatitis services, TB screening, mental health, substance use treatment, and other supportive services as needed.

Promote routine and opt-out HIV testing, HIV self-testing, and other HIV prevention and testing strategies in healthcare and community settings.

Bachelor’s degree in a related field from an accredited college or university and two (2) years of job-related experience; General knowledge of HIV/AIDS care processes, Ryan White eligibility requirements and community resources, understanding of the unique needs of populations most affected by HIV, and lived experiences related to HIV care or prevention services. General office environment with multiple appointments, phone calls, and emails daily. will require some walking and long periods of standing during field work and outreach events. Requires flexibility of working in general office environment, clinic environment, and community outreach. Local travel and occasional overnight training/travel possible.
