Physical Therapist - Full Time - Home Health Care

Move Mend Clinic is seeking a dedicated and skilled Physical Therapist to provide high-quality therapeutic care in a home health setting. This role is performed onsite, focusing on home care delivery, which allows our therapists to work directly in the patients' environments. This setting not only fosters a more personalized approach to therapy but also enables you to observe and address the specific challenges patients face in their daily lives. Conduct patient screenings, evaluations, and diagnostics to identify physical therapy needs and develop comprehensive treatment plans.

Utilize evidence-based physical therapy modalities and techniques tailored to a variety of patient diagnoses, including but not limited to musculoskeletal injuries, neurological disorders, and post-surgical recovery.

Provide education and support to patients and caregivers, helping them understand treatment protocols and encouraging self-management strategies.

Create and maintain accurate and timely documentation of patient evaluations, progress notes, treatment plans, and billing information, complying with regulatory and facility standards.

Ensure high standards of care by collaborating with a multidisciplinary team, including physicians, nurses, and rehabilitation staff, to discuss treatment goals and update care plans as needed.

Engage in continued professional development through workshops, webinars, and other educational opportunities to stay current with best practices and advances in physical therapy.

Participate in quality improvement initiatives, contributing to departmental goals, patient satisfaction metrics, and clinical outcomes.

Tailoring your approach to different age groups and conditions is essential, enabling you to focus on functional improvements that impact our patients' quality of life and independence.

A current and active Physical Therapist license in the state of New Jersey.

A Bachelor's degree in Physical Therapy; At least one year of clinical experience, ideally within a healthcare setting that specializes in home or community-based therapy.

Proven ability to perform essential physical therapy tasks, including assessment, treatment planning, and exercise prescription while demonstrating effective patient communication.

Experience working in a home care environment is highly valued.

Familiarity with electronic health record systems for documentation and patient management.
