The primary purpose of your job position is to plan, organize, and implement social services to PACE Organization

provision of resources, addressing mental health needed, ongoing case management, advocacy to ensure participant

From assessment to counseling, education to advocacy, you'll be at the forefront of ensuring holistic care and support.
Master's degree in social work coupled with two years of relevant experience.
Preferably licensed in social work within our state.
Experience in geriatric social work is highly valued.
Proficiency in social service principles and funding mechanisms.
