Home Care/Hospice RN Liaison

Gentiva is seeking an RN Clinical Liaison to support patients transitioning to home-based care. In this role, you will educate patients and families on Gentiva’s home health and hospice services, assess clinical needs, coordinate referrals, and collaborate with hospitals, physicians, and care teams. You will complete timely assessments, develop care transition plans, and ensure smooth admissions to home-based services. The RN Clinical Liaison serves as a clinical resource and advocate, promoting compassionate, dignified care while building strong community and provider relationships in a mission-driven environment.
