Community Living Center Registered NurseThe Community Living Center (CLC) Registered Nurse is responsible for providing and coordinating high-quality, evidence-based nursing care for residents in a long-term and rehabilitative care setting. This role is accountable for implementing all phases of the nursing process, ensuring safe administration of medications and procedures, and promoting patient education, self-management, and satisfaction. Deliver and oversee direct patient care, ensuring adherence to established policies, guidelines, and evidence-based practices.Collaborate with interdisciplinary team members to support optimal patient outcomes and continuity of care.Participate in unit and program quality improvement initiatives and support customer service goals.Supervise and guide licensed vocational nurses and nursing assistants, ensuring appropriate delegation and oversight.Promote a holistic, patient-centered care environment through effective communication and teamwork.Support evidence-based practice initiatives and collaborate with the Minimum Data Set (MDS) coordinator to ensure accurate documentation and Resource Utilization Group (RUG) scoring.Identify opportunities to enhance efficiency, timeliness, and cost-effective care delivery within the clinical setting.Monitor and evaluate care outcomes to drive continuous improvement.Advocate for patients, families, and caregivers to support safe, coordinated, and satisfactory care experiences.Apply high-reliability principles and I CARE values to ensure consistent, high-quality care delivery.Two years of RN experience in long-term care, hospice, urgent care, or primary care.
