Home Health Aide/Caregiver
You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on -- providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
Maintain a clean, safe, and orderly client environment
Use safety equipment properly (rails, supports, stools, etc.)
Document care provided according to company and regulatory standards
Travel reliably to patient homes and assigned visits across your service area
Complete required in-service training, including annual dementia education
Accurate, timely care documentation
Completion of required certifications and training on schedule
Strong communication with supervising nurse and care team
Active HHA or CNA certification or willingness to obtain certification
Ability to follow written care instructions and document services
Reliable transportation and ability to travel to client homes
Willingness to complete TB testing, background screening, and onboarding requirements
6+ months of caregiving or direct patient care experience

• Home health or home care experience
• CPR and Adult First Aid certification
• Experience working with dementia or memory care patients

One-on-one care in patient homes and occasional facility settings
Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
Travel within your preferred coverage area
