The Registered Nurse (RN) - Wound Care Nurse provides specialized wound, skin, ostomy, and continence care to patients in their homes. The RN is responsible for assessing wounds and skin integrity, developing and implementing care plans, monitoring healing progress, educating patients and caregivers, and collaborating with the healthcare team to achieve optimal patient outcomes.

Evaluate patient skin condition and wound care needs upon admission and on an ongoing basis

Develop, implement, and update individualized wound care plans based on clinical findings and physician orders

Establish measurable goals and recommend appropriate nursing interventions to promote healing and prevent complications

Perform wound care treatments and dressing changes according to physician orders and evidence-based practices

Educate patients, family members, and caregivers on wound management, skin integrity, infection prevention, and treatment plans

Collaborate with physicians and other healthcare professionals regarding wound care recommendations and patient progress

Communicate significant changes in patient condition to the Director of Nursing and other appropriate care team members

Adhere to agency policies, infection control standards, and nursing practice regulations

Current, unrestricted Texas Registered Nurse (RN) license

Minimum one (1) year of documented nursing experience in a home health agency, primary care clinic, wound care clinic, hospital, or healthcare facility preferred

Wound, Ostomy, and Continence Nursing Certification (WOCN) preferred

Knowledge of evidence-based wound care practices and skin integrity management

Ability to stand, walk, bend, squat, kneel, reach, twist, push, and pull throughout the workday

Ability to safely assist patients as needed during care visits
