Day Center CNA

Salary Range $19.20 - $22.08 Hourly Position Type Full Time

A CNA is responsible for assisting participants in the program for all inclusive care for the elderly (PACE) with activities of daily living (ADLs), such as showering, eating, dressing, etc. They also assist in the delivery of instrumental activities of daily living (IADLs) and restorative care, such as accompaniment to clinic appointments, physical therapy exercises and quality of life activities. Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed

Creates and participates in group(s) and one-on-one activities in memory impaired unit

Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas

Assists the drivers with the loading and unloading of participants from vehicles

Participates in participant's care planning through cooperation with interdisciplinary teams

May assist other departments as a CNA as approved by the Day Center Manager

Documents services provided to participants consistently, accurately and in a timely manner via Electronic Health Record (EHR). Participates in participant's care planning as requested

Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.

Participates in Quality Assurance and program improvement initiatives as assigned

High School Diploma or equivalent. Must be licensed as a Certified Nursing Assistant (CNA) in the state of Colorado

Current First Aid/BLS Certification is required

Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers

Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records

Ability to work independently with minimum supervision
