Oasis Hospice And Palliative Care Inc. is seeking a compassionate and dedicated On Call RN to join our team in Homewood, IL. In this vital role, you'll provide expert after-hours nursing care and support to patients and families during some of life's most meaningful moments. Conduct urgent and unscheduled home visits to evaluate and manage patient symptoms

Administer medications and treatments in accordance with the patient's plan of care

Communicate effectively with the interdisciplinary hospice team regarding patient status changes

Document all clinical assessments, interventions, and communications accurately and timely

Coordinate with physicians and care team members to adjust care plans as needed

Active RN license in the state of Illinois

Previous hospice, palliative care, or home health nursing experience preferred

Excellent communication and interpersonal skills with patients, families, and care teams

Compassionate, patient-centered approach to end-of-life care
