Advanced Practice Hospital Nurse

SUMMARY

Under the general supervision of the Nurse Manager (or designee), the professional registered nurse acts as a patient advocate in applying the nursing process. All essential functions performed in this position reflect age‑specific needs of the patients.

RESPONSIBILITIES

Recognizes and reports deviations from patient or established norms.

Identifies deviations from established norms not related to the chief patient’s complaints.

Reports patient responses to treatment(s) and documents nursing interventions.

Provides basic comfort and support according to patient needs.

Fosters patient and family participation in achieving optimal functioning and daily living activities.

Sets priorities in individual patient assignments.

Seeks guidance and resources for management of patient care when necessary.

Identifies and assesses emergency situations.

Initiates appropriate response and intervention during emergencies.

Plans for potential needs during emergency situations.

Defines nursing diagnoses and expected patient outcomes.

Modifies the nursing plan of care based on diagnoses and/or outcomes.

Documents assessments on appropriate nursing forms.

Implements nursing interventions and evaluates them based on patient outcomes.

Adjusts the plan or care accordingly, including documentation.

Utilizes established protocols and collaborates with the health team to facilitate therapeutic interventions and desired outcomes.

Participates in quality improvement processes and risk management activities.

Attends unit‑based or hospital meetings to facilitate patient‑care management.

Functions cooperatively and productively in the work environment.

Integrates comprehensive physical assessment with nursing assessment, reviewing records to develop a comprehensive, individualized care plan.

Follows all documentation guidelines per policy and procedure.

Maintains quality assurance, safety, environmental, and infection‑control standards.

Provides a safe environment for patients, visitors, and staff.

Demonstrates self‑direction in maintaining and advancing clinical and professional competency.

Provides guidance and direction to LPNs, CNAs, and other licensed personnel as required.

Assumes shift‑charge responsibility on an assigned unit when required, including bed and patient assignment and task delegation.

QUALIFICATIONS

Licensure as a Registered Nurse in the State of Rhode Island or pursuant to the Nurse Licensure Compact.

Graduate of an accredited nursing school; Associate’s degree required, Baccalaureate preferred.

Current Basic Life Support (BLS) certification (American Heart Association).

Conforms to the Code of Professional Nurses.

Understands the provisions of the Nurse Practice Act of Rhode Island.

Demonstrates knowledge and skills necessary to provide care throughout the life span, considering aging processes, human development stages, and cultural diversity.

EXPERIENCE

Registered nurses should develop general proficiency as a staff nurse within three months of employment. For specialty units, general nursing experience appropriate to the specialty is preferred.

PHYSICAL REQUIREMENTS

Prolonged standing and walking.

Exposure to all patient elements.

Repeated bending, squatting, lifting, and stooping.
