Home Health Hospice Case Manager
Job Summary
We are seeking a skilled Home Health Hospice Case Manager with experience in the home health setting to provide compassionate end-of-life care. The ideal candidate will possess a strong understanding of hospice principles and demonstrate the ability to deliver high-quality nursing care in patients' homes.
Responsibilities
Conduct comprehensive assessments of patients and develop individualized care plans.
Coordinate and collaborate with multidisciplinary teams to ensure optimal patient outcomes.
Provide education and support to patients and their families regarding hospice services.
Utilize effective communication skills to ensure clear and ongoing conversations with patients and families.
Monitor patient progress and modify care plans as necessary.
Document all nursing assessments, interventions, and patient progress accurately in compliance with relevant standards.
Qualifications
Current licensure as a Registered Nurse (RN).
Experience in hospice care within a home health setting is required.
Strong assessment and critical thinking skills.
Excellent interpersonal and communication abilities.
Ability to work independently and manage a caseload effectively.
Additional Information
Day shift available, 5 days a week (08:00 - 16:30).
Mileage will be reimbursed at the IRS rate.
No on-call responsibilities.
