Certified Nursing Assistant (CNA) / Hospice Aide

At Moments Hospice, we believe every patient deserves comfort, dignity, and compassionate care in the final chapter of life. As a hospice CNA, you will provide the hands-on personal care that helps patients feel comfortable, clean, and cared for wherever they call home, whether that's a private residence, assisted living, or a care facility. You'll work closely with a dedicated RN Case Manager as a valued member of our interdisciplinary care team, in a supportive setting with clear expectations and reasonable caseloads. With structured training and room to build your skills, you'll grow your career as part of our growing, collaborative team.

Our fleet car program gives you a company vehicle for both business and personal use, with gas and insurance covered, at minimal cost to you. Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care

Assist with dining and nutrition

Review and follow each patient's plan of care under the direction of the RN Case Manager

Maintain electronic medical records

Active Certified Nursing Assistant (CNA) certification (required)

Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus

Ability to work in an electronic medical record system

Excellent customer service, problem-solving, and time management skills

Compassion, patience, and the ability to handle emotional situations with care

Valid driver's license and current auto insurance (required)
