Description

Conduct evaluations for patients in both inpatient and outpatient settings and craft tailored treatment plans.
Provide physical therapy interventions concentrating on mobility, strength, balance, and functional independence.
Work closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes.
Educate patients and their families on therapeutic goals, discharge strategies, and home exercise plans.
Maintain accurate documentation of evaluations, treatments, and progress in line with facility and regulatory requirements.
Engage in care team rounds and interdisciplinary meetings to promote cohesive patient care.
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program.

Certifications
State licensure as a Physical Therapist
