Hospice Triage Registered Nurse (RN)
The Hospice Triage Registered Nurse (RN) receives incoming triage calls from patients and providers, addressing changes in physician orders and patient care needs as appropriate.

Position is in-person, located at the Hospice House.
Weekend Hours: Saturday and Sunday, 8am to 8:30pm.
Assists patients, families, clinical staff and providers via telephone with clinical issues, problems and questions.
Participates in case conferences and interdisciplinary team meetings and coordinates changes in physician orders with clinical staff and providers.
May provide nursing visits as required to deliver patient care according to home health policies.
Receives physician orders, providing follow-up with appropriate clinical staff; Associate's Degree Accredited Registered Nurse Program- Required
Registered Nurse (RN)- State Licensing Board - Required
Auto Insurance - Required
1 year- Hospice Experience- Preferred
