Be rewarded with a unique opportunity to make a differenceCompetitive compensation packageTuition reimbursement for full-time staff and continuing education opportunities for all employees at no costOpportunities for advancementComprehensive insurance plans for medical, dental, and vision benefits401(K) with employer matchPaid time off, paid holidays, family, and pet bereavementAs a Licensed Practical Nurse Behavioral Health you'll contribute to our success in the following ways:Verifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physician's orders, assessment data, and established standards and guidelines.Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes.Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate.Implements the plan for patient safety, using the patient, family, and community resources.Assists the patient in learning appropriate self-care techniques while delivering quality patient care.Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician.Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.Documents medication regimen and updates medication profile according to established procedures.Implement all available actions to prevent avoidable hospitalizations and ER visits.Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.High School Diploma or GEDGraduate of an accredited Certificate, Diploma, or Associate School of NursingCurrent State License as a Licensed Vocational / Practical Nurse1 year of experience in a clinical care settingExperience in a hospice or home health environment is preferred50% travel requiredAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmentValid driver's license and insurance and reliable transportation to perform job tasksYou will report to the Clinical Team Manager.Individuals are recruited, hired, assigned and promoted without regard to race, national origin, religion, age, color, sex, sexual orientation, gender identity, disability, protected veteran status, or any other protected characteristic.
