On-Site Registered Nurse

Partner and collaborate during the initial evaluation to assess residents' needs, helping to develop and implement a personalized service plan in coordination with the resident, family, physician, and/or authorized practitioner

Monitor and communicate any changes in a resident's condition

Ensure each resident's individualized care plan is effectively implemented and consistently followed by the Home Care team

Deliver nursing care in accordance with the established plan of care and accepted nursing standards

Participate in care conferences and other resident-related meetings to support coordinated, high-quality care

Provide clinical oversight by developing and supervising the implementation of individualized service plans, continuously observing residents' conditions, and communicating updates to physicians and families as needed

Lead and guide the Home Care teamincluding LPNs, aides, home care workers, and companionsto ensure safe, effective, and person-centered care delivery

Must have a minimum of 6 months' experience as an RN

Basic computer skills required including experience with Microsoft Office, internet and web applications

Experience with an electronic medical record is preferred
