Evolve Physical Therapy IL is a concierge physical therapy practice specializing in custom recovery solutions tailored to each individual’s goals. As a private practice, it provides one-on-one orthopedic and sports medicine care in a focused, supportive setting. The team is dedicated to helping clients return to the activities and pursuits that matter most in their lives. Evolve emphasizes personalized attention, comprehensive treatment plans, and high-quality, evidence-informed care throughout each client’s recovery journey.
This is a full-time, on-site Outpatient Physical Therapist role based in Aurora, IL. The Outpatient Physical Therapist will independently evaluate patients with orthopedic and sports-related conditions, develop individualized plans of care, and deliver one-on-one treatments using manual therapy, therapeutic exercise, neuromuscular re‑education, and patient education. Daily responsibilities include documenting care in a timely and accurate manner, collaborating with referring providers and other healthcare professionals, and updating treatment plans based on patient progress. The therapist will also educate patients on injury prevention, home exercise programs, and long‑term wellness strategies, while contributing to a positive, patient-centered clinic environment.
Licensed (or license-eligible) Physical Therapist in the state of Illinois; Doctor of Physical Therapy (DPT) or equivalent from an accredited program.
Strong clinical skills in orthopedic and sports rehabilitation, including evaluation, manual therapy, therapeutic exercise prescription, and movement analysis.
Solid documentation and time-management skills, with proficiency in or willingness to learn electronic medical record (EMR) systems.
Interest in ongoing professional development, including continuing education in orthopedic, sports, or manual therapy specialties; Ability to work on-site full time in Aurora, IL, including occasional flexibility in schedule to meet patient needs.
