Licensed practical nurse health care

Position Overview:
We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health nursing services to patients in their homes. The ideal candidate will deliver high-quality patient care, develop individualized care plans, collaborate with interdisciplinary teams, and ensure compliance with Medicare regulations, state requirements, and agency policies.
Key Responsibilities:

Perform comprehensive in-home nursing assessments.
Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, and Discharge visits.
Develop and update individualized patient care plans.
Provide skilled nursing care, including medication management, wound care, catheter and ostomy care, injections, IV therapy (if applicable), and chronic disease management.
Educate patients and caregivers on treatments, medications, and disease management.
Collaborate with physicians, therapists, case managers, and other healthcare professionals.
Supervise Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs) as needed.
Maintain accurate electronic documentation.
Ensure compliance with HIPAA, Medicare guidelines, and state home health regulations.
Participate in quality improvement initiatives and team meetings.
Required Qualifications:

Active Indiana Registered Nurse (RN) license in good standing.
Minimum of 2 years of nursing experience.
Current CPR/BLS certification.
Valid driver's license with reliable transportation and proof of auto insurance.
Strong communication, organizational, and time management skills.
Ability to work independently in a field-based environment.
Preferred Qualifications:

Home Health nursing experience.
OASIS documentation experience.
Medicare Home Health experience.
Experience with Electronic Medical Records (EMR).
Leadership or supervisory experience.
Bilingual candidates are encouraged to apply.

Monday-Friday
Day Shift
Weekends as needed
