Seeking a dedicated and compassionate Physical Therapist to join a growing home health team. This role is ideal for clinicians passionate about patient-centered care who enjoy working with diverse populations in a flexible and supportive environment.

Responsibilities:

Perform comprehensive physical therapy evaluations and assessments

Develop and implement individualized treatment plans based on patient goals

Deliver hands-on therapy using evidence-based techniques

Educate patients and caregivers on exercises, safety, and recovery strategies

Collaborate with interdisciplinary healthcare professionals to ensure coordinated care

Maintain accurate and timely documentation using EMR systems

Participate in discharge planning and continuity of care

Patient Populations:

Adults and Pediatrics

Geriatrics

Orthopedics

Home and community-based care

Qualifications:

Licensed Physical Therapist or eligible for licensure

Strong knowledge of anatomy, physiology, and rehabilitation principles

Experience in outpatient, inpatient, or home health settings preferred

Ability to work independently in a home health environment

Excellent communication and patient education skills

Position Details:

Caseload of 3-5 patients per day

Flexible scheduling including day, night, and on-call shifts

Weekend availability optional and preferred

In-person, home-based care setting
