• The Licensed Practical Nurse is responsible for providing general nursing care to patients, under the direction of a Registered Nurse and in accordance with physician’s orders and established standards and protocols. Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.

Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
Reports to supervisor on patient care rendered, problems concerning patients’ needs, and resolution to those needs.
Helps identify patient problems and potential risks and contributes to the development of interventions.
Assists in identification of patient’s learning needs and reinforces self-care and other techniques.
Appropriately designates applicable charges to individual patients using the hospital’s established system.
May be required to work during inclement weather and other staffing emergencies.
Assesses the risks for safety and implements appropriate precautions.
Complies with appropriate and approved safety and Infection Prevention standards.
Two years’ experience in inpatient hospital setting, inpatient rehabilitation experience preferred.

Current state license as a Licensed Practical Nurse required.
Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients.
Demonstrates general computer skills including data entry, word processing, email, and records management.
Ability to maintain quality, safety, and/or infection prevention standards.
Physical Requirements Over the Course of a Shift:

Visual acuity required for patient assessment and documentation of care.
Sufficient manual dexterity to operate equipment and computer keyboard.
