Caregivers / Home Health Care - Part Time

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Life Skills Trainer / Caregiver – Full Time

Schedule: Monday – Friday | Day Shift | Part Time

As a Life Skills Trainer / Caregiver , you’ll play an important role in helping individuals build independence and live fulfilling lives. This is hands-on, meaningful work where your compassion and support make a lasting impact.
Provide training, assistance, and supervision in residential and community settings
Assist with daily living activities such as meal preparation, personal hygiene, shopping, cleaning, and medication administration
Encourage community involvement by accompanying individuals to outings, activities, or work
Help maintain positive relationships between individuals served and their employers
High School Diploma or GED
Must be 18 years of age or older
Six months to one year of experience in the human services field preferred
