Description

Assess patients in both inpatient and outpatient settings, crafting personalized treatment strategies
Deliver physical therapy interventions aimed at improving mobility, enhancing strength, and optimizing balance and function
Collaborate closely with nurses, physicians, case managers, and other rehab experts to maximize patient outcomes
Educate patients and their families on therapeutic objectives, discharge processes, and at-home exercise regimens
Maintain thorough documentation of evaluations, treatments, and patient progress in accordance with facility protocols
Engage in care team meetings and interdisciplinary discussions to ensure smooth and effective patient care
Education

~ Doctorate in Physical Therapy (DPT) or a comparable qualification from an accredited institution

Certifications

State licensure as a Physical Therapist
Preferred CPR certification
Specialty certifications (i.e., neurological, geriatric) are advantageous but not mandatory
Skills

Prior experience is highly recommended
Proficient in treating patients with complex medical and mobility challenges
Exceptional communication and teamwork abilities
Adept at using digital documentation and clinical record systems
