Schedule: Monday thru Friday, 8am-5pm
Are you a Registered Nurse looking for a new opportunity? Hospice of North Alabama is seeking a passionate, dedicated Hospice Admissions RN to join our team in Scottsboro, AL . Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. Provide 1:1 care to make a lasting impact on patients and families

As a Hospice Admissions RN You will:
Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for h ospice care
Explain hospice services, discuss goals and care with the patient and families , answer any questions, and obtain necessary consents and legal documents
Obtain medical history, current medications, and symptoms
Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
Coordinate the start of services (nursing, aides, equipment, medications.)

*Graduate of an accredited nursing school with current licensure to practice in state of operation
One year of hospice nursing experience required , 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
