Responsibilities Provide patient care and support in home settings.
Collaborate with multidisciplinary teams to manage and deliver patient care plans.
Use tablets, mobile phones, and EMR software to document care and communicate with team members.
Self-manage time and schedule, including approximately 20% travel between patient locations.
Qualifications Certified Nursing Assistant certification in the state where you work.
High School Diploma or equivalent.
Minimum of one year of healthcare experience.
