Licensed Vocational Nurse (LVN) Home Health Shifts

Job Summary: The Licensed Vocational Nurse (LVN) in a Skilled Nursing Facility (SNF) provides individualized nursing care in collaboration with physicians, nursing staff, residents, families, and other departments. The LVN conducts patient assessments, administers treatments and medications, documents care, and responds to emergencies while maintaining professional conduct and communication. This role requires adaptability, strong clinical skills, and the ability to work under the direction of the Director of Nursing and Charge RN to ensure high-quality patient outcomes in an acute and skilled nursing environment.

Conduct individualized patient assessments and notify Charge Nurse and physician of abnormal findings.

Develop and implement individualized patient care plans in collaboration with the healthcare team.

Document patient assessments, nursing interventions, medication administration, and progress in patient records.

Respond promptly and appropriately to emergency situations, including initiating resuscitation measures and notifying relevant personnel.

Assist in coordinating patient transfers, diagnostic tests, and specimen collections.

Support nursing staff in other departments as needed and assist with orientation and education of new personnel.

Adhere to safety, infection control, and confidentiality policies and procedures.

Participate in end-of-shift walking rounds and communicate patient status during shift changes.

Perform other duties as assigned by nursing management.

Current California LVN license in good standing.

Strong physical health and stamina to perform patient care duties including walking, bending, and lifting.

Ability to communicate effectively in English, both verbally and in writing.

Good mental health and ability to manage challenging patient behaviors professionally.

Ability to use Microsoft Office applications and office equipment.

Experience working in skilled nursing, acute care, or related healthcare settings.

Experience assisting with orientation and education of new nursing staff.

Must wear designated Caribbean Blue scrubs as a uniform requirement.

Adherence to facility policies regarding confidentiality and professional conduct.

Work hours and shifts are determined based on the needs of the department.

Participation in scheduled end-of-shift walking rounds at 0630 and 1830 to ensure continuity of care.

Flexibility to work various shifts and possibly extended hours as required.
