What You’ll Do & Bring As a Licensed Vocational Nurse (LVN) , you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with physician orders and individualized Plans of Care. You will observe and assess patients, monitor progress, provide treatments within the scope of LVN practice, and educate patients and caregivers to support optimal health outcomes, independence, and quality of life. The LVN plays a vital role in coordinating care, promoting patient safety, ensuring compliance with agency policies and Medicare and Medicaid regulations, and collaborating with the interdisciplinary care team to deliver compassionate, patient-centered care.

Job Summary Provide nursing care to patients in the home setting. Observe and assess the patient and/or caregiver to enhance quality of life and promote positive health outcomes. Demonstrate individualized creativity in educating patients and caregivers while following agency nursing policies and procedures. Provide care in accordance with physician orders and the established Plan of Care. Duties and Responsibilities Provide skilled nursing care to patients in accordance with physician orders, the Plan of Care, and agency policies.

Work under the direction and supervision of a Registered Nurse (RN).

Demonstrate effective teamwork and collaboration with clinical and administrative staff.

Perform nursing procedures according to agency policies and procedures.

Monitor for changes in patient condition and promptly report adverse findings to the RN and physician.

Administer medications and treatments as prescribed and within the scope of LVN practice.

Educate patients and family members regarding disease processes, medications, treatment options, home care procedures, safety practices, and the Plan of Care.

Reinforce teaching and care instructions established by the physician and RN.

Coordinate and monitor patient care and services with members of the interdisciplinary team.

Maintain patient records according to agency policies and procedures.

Ensure compliance with HIPAA regulations both in and outside the office.

Follow infection control practices in all work settings.

Support quality improvement initiatives and agency performance goals.

Commitment to serving as a patient advocate for quality healthcare and services.

Ability to respond effectively and professionally to sensitive inquiries or complaints.

Ability to manage multiple responsibilities and projects simultaneously.

Strong team player with a collaborative approach to patient care.

Ability to read, analyze, and interpret complex healthcare documents and regulations.

Required Education and Experience Graduate of an accredited Licensed Vocational Nursing (LVN) program.

Current Texas Licensed Vocational Nurse (LVN) license in good standing.

Minimum of two (2) years of experience in a healthcare setting required.

Home health experience preferred.

Knowledge of home health regulations, standards of practice, Medicare, and Medicaid guidelines preferred.

Reliable transportation and ability to travel throughout assigned service areas.

Work Location & Travel Requirements Home Office Location: MyCHN Home Health Services

This position requires regular travel to patient homes and assigned locations throughout Brazoria, Galveston, and Harris Counties .
