Schedule: Flexible PRN coverage (weekdays & weekends)

Shifts:

Monday: 6:30a 4p, 10a 9p, 3p 12a, 9p 7a

Tuesday: 6:30a 4p, 10a 9p, 4p 12a, 9p 7a

Wednesday Friday: 6:30a 4p, 11a 9p, 4p 12a, 9p 7a

Saturday & Sunday: 7a 4p, 11a 9p, 4p 12a, 9p 7a

Assignment Details:

Physician Requirements:

BC/BE EM, FM, or IM with 2+ years of EM experience

Active OH license & DEA, or IMLC eligible with ability to obtain quickly

CAQH attestation required

Submission / Credentialing:

No temps allowed
90 day privileging once app is submitted
Required items: completed application, COI, credentials, education/training/licensure verification, 5 year claims history, full work history, 3 peer references
Documents needed: state license & DEA, degree, residency certificate, board certificate, photo, driver s license, TB, flu
ABEM: no ongoing cert maintenance; must show ATLS once in the past
ABFM/ABIM: must maintain ATLS
