Physical Therapist Assistant (PTA) - Home HealthWe are seeking a dedicated and experienced Physical Therapist Assistant (PTA) for a home health position. The ideal candidate should have recent or current home health experience and be proficient with the Homecare Homebase charting system.ResponsibilitiesProvide physical therapy interventions under the direction of a licensed Physical Therapist.Assist patients with rehabilitation exercises and mobility activities.Monitor and document patient progress accurately using the Homecare Homebase charting system.Collaborate with healthcare team members to ensure quality patient care.Educate patients and their families about the rehabilitation process and self-care techniques.Required Experience / Certifications / LicensureMinimum of 6 months of home health experience required.Tennessee PTA license or compact required.COVID-19 vaccine is required, with medical/religious exemptions only.Flu vaccine is required, with medical/religious exemptions and declinations allowed.ScheduleThis position offers a day shift, from 8:00 AM to 4:00 PM, with a schedule of 5 days per week (5x8 hours).
