Maintains competencies within practical nursing standards and seeks opportunities to develop professionally. Provides care to patients whose care requirements are routine/standardized and with predictable outcomes./pulliFollows the seven rights of medication administration while reporting adverse reactions to the physician immediately and taking action in emergency cases./liliCoordinates referrals for patient chronic care conditions and follow up care as indicated by the initial care plan of the specialist and/or primary care physician./liliAdministers prescribed oral and intramuscular medications, therapies and treatments following practical nursing standards and under the direct supervision of the physician./liliEffectively communicates with healthcare team members, patients and families; supports healthcare team members to achieve team goals and assigns appropriate tasks to care assistants./liliMaintains and reviews patient medical records, notes and physician orders; documents all patient care activities in accordance with documentation standards and legal requirements for medical facilities./liliUpholds the standards of Patient-Centered Medical Home (PCMH) and assists other clinical staff members in completing and following quality measures under PCMH and Patient-Centered Specialty Practice (PCSP)./liliCreates a plan of care that is patient-centered and provides a written copy of the plan of care to patients./liliProvides direct patient care, utilizing the nursing process, under the direction and supervision of the physician./li/ulpRequirements:/ppCertification - Basic Life Support/ppLicensure - Licensed Practical Nurse licensed or eligible for licensing in the state of employment as a Licensed Practical Nurse/ppExperience - 2 years of nursing practice in a hospital or clinic setting/ppEducation - Vocational/Technical Graduate of an accredited Licensed Practical Nurse school/ppKSAs - Technology use; Effective communication/ppPreferred experience: clinical experience in outpatient medical office setting/ppAbout Us:/ppWe are Inova, Northern Virginias leading nonprofit healthcare provider.
