From managing complex medical conditions and monitoring changes in patient status to supporting mobility, preventing complications, educating families, and coordinating care across the continuum, every day brings a new opportunity to make a difference.

Prioritize, coordinate, and delegate all aspects of patient care while managing the unique and often complex needs of senior adult patients.

Work closely with physicians, advanced practice providers, therapists, case managers, pharmacists, and other multidisciplinary team members to deliver exceptional, patient-centered care.

Maintain and promote a safe environment for patients, families, visitors, and team members, with a strong focus on fall prevention, mobility, skin integrity, infection prevention, and other age-specific safety needs.

Monitor and engage with patients, families, and appropriate care team members regarding significant events, changes in patient condition, questions, concerns, and individualized care needs.

Champion quality and infection prevention. Monitor and execute Joint Commission, infection control, and environment of care requirements to support safe, high-quality patient care.

Keep care moving. Ensure continuity of care, efficient patient throughput, consistent communication, and an exceptional experience for every patient and family.

Initiate patient and family education using appropriate resources and continuum-of-care services, helping patients and caregivers understand diagnoses, treatments, medications, discharge plans, and next steps.

Graduate of an accredited school of nursing.

Current Registered Nurse license in the applicable state.
