PRN Home Health Registered Nurse
Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Registered Nurse to provide skilled nursing care for patients in their homes throughout the Gainesville and Ocala area. The schedule for the RN PRN is 5-10+ visits per week and some weekend/holiday on-call rotations.

What You'll Do

Perform comprehensive patient assessments, including OASIS assessments at required intervals
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care
What You'll Need

Graduate of an approved nursing program with an active Florida RN license
Prefers BSN or MSN degree
Requires home health RN experience.
Prefers experience using Homecare Homebase (HCHB) EMR
Strong assessment, critical thinking, and patient education skills
Willingness to participate in weekend and holiday on-call rotations
