Licensed practical nurse rehabilitation and nursing care center

The position involves working in a long-term care facility. The census typically ranges from 80 to 95 residents. The facility consists of shared rooms, with private room availability limited. Experience with geriatric patients is preferable. Common medical diagnoses include feeding tubes, IV antibiotics, post-surgical care, and rehabilitation. Job duties include administering IVs, conducting assessments, managing medications, performing dressing changes, and using wound vacs. Registered Nurses will complete admissions and discharges. Certified Nursing Assistants will perform vital signs and assist with activities of daily living. The nursing staff will manage IV antibiotics and fluids; however, there are no titratable drips. The interdisciplinary team includes various specialists such as physical, occupational, and speech therapists, social workers, and dietitians. Onsite pharmacy services are available 24/7. Schedule management will try to accommodate block scheduling for shifts. Weekend and holiday work are required on an alternating basis. Staff may float between units as needed, though efforts will be made to maintain consistency within the unit during a stretch of shifts. Professional scrubs are required in any color. COVID units are intermixed within the facility. Both N95 masks and PAPR are utilized. Charting is completed via Point Click Care (PCC).
