• 9:00am-6:00pm and 1 weekend per month rotation \n
• 10:00am-7:00am and 1 weekend per month rotation \n
• 11:00am - 8:00pm and 1 weekend per month rotation \n

• n What You’ll Do

Conduct comprehensive clinical assessments to determine hospice eligibility \n
Educate patients and families about hospice services and Medicare benefits \n
Collaborate with physicians, case managers, and interdisciplinary teams \n
Provide emotional support and clinical guidance during the decision-making process \n
Initiate and complete the admission process, including documentation and coordination \n

• n What We’re Looking For

Registered Nurse (RN) with a valid state license \n
Minimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred)\n
Strong clinical assessment and critical thinking skills \n
Compassionate communicator with excellent interpersonal skills \n
Active listener who builds trust and rapport \n
Confident in guiding others through uncertainty, addressing concerns with empathy \n
Reliable professional with strong follow-through and autonomy \n
Patient-centered advocate committed to dignity and comfort \n
