Responsibilities Conduct meaningful assessments to understand residents’ needs, strengths, and goals.

Develop individualized treatment plans that honor each resident’s story.

Connect residents with vital community resources, including mental health services, housing assistance, and support programs.

Collaborate with nursing, therapy, and interdisciplinary teams to ensure holistic, person‑centered care.

Maintain accurate, confidential records that reflect the highest standards of professionalism.

Participate in care conferences and team meetings to advocate for residents’ needs.

Stay informed about community programs to better serve residents and families.

Qualifications Must hold an MSW (Master of Social Work) from an accredited institution.

At least 2 years of experience in healthcare, LTCS/NF, geriatrics, behavioral health, or case management.

Strong understanding of social work theories, interventions, and techniques.

Heart for serving individuals from diverse cultural and socioeconomic backgrounds.
