Overview

We are seeking a Physical Therapist to provide personalized 1:1 rehabilitative therapy in patients’ homes and communities. The role involves conducting comprehensive assessments, developing treatment plans, delivering therapy, and adjusting care based on patient progress. The therapist practices independently within an interdisciplinary care team, ensuring patient needs are met effectively.

Key Responsibilities

Perform assessments and develop individualized treatment plans.

Deliver rehabilitative therapy tailored to each patient’s needs.

Monitor patient progress and modify plans as necessary.

Practice independently within the community setting.

Qualifications & Skills

Current New York State Physical Therapy license and registration.

Bachelor’s Degree in Physical Therapy from an approved program.

Minimum one year of physical therapy experience or successful completion of a rehab intern program.

Basic PC skills; bilingual skills may be advantageous.

Valid driver’s license may be required.
