Base Pay: $25.00-$29.55 per hour (based on experience)
Shift: 3:00 PM-11:00 PM

Hunter Creek Health & Rehabilitation is excited to welcome Licensed Practical Nurses (LPNs) to our growing team as we begin an exciting new chapter under the leadership of a new Administrator and Director of Nursing.

We're seeking a compassionate and dependable Licensed Practical Nurse (LPN) Charge Nurse to provide exceptional resident care while leading the nursing team during the 3:00 PM-11:00 PM evening shift . Licensed Practical Nurse (LPN) – Charge Nurse Responsibilities
Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
Assess, monitor, and document changes in resident conditions.
Administer medications, treatments, and nursing interventions according to physician orders.
Lead and supervise Certified Nursing Assistants (CNAs) while supporting quality resident care.
Coordinate resident care with Registered Nurses (RNs), physicians, therapists, and the interdisciplinary healthcare team.
Respond promptly to changes in resident condition and communicate clinical concerns appropriately.
Maintain accurate electronic nursing documentation in compliance with state and federal regulations.
Promote infection prevention, resident safety, and quality improvement initiatives.
Foster a positive, supportive, and team-oriented work environment.
Current Licensed Practical Nurse (LPN) license in good standing.
Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
Previous Charge Nurse or leadership experience is preferred but not required.
