Entyre Care helps elders and peoples with disabilities stay and health at home with trusted caregivers. As an RN Supervisor, you are the clinical backbone of that model — you assess clients in their homes, build their plans of care, and train and mentor their caregivers.

This is hands-on, in-the-field nursing with real autonomy. You will report to the Program Director and help shape how a growing agency delivers care.

This role serves as the operational bridge between members, family caregivers, managed care organizations, and Entyre’s internal teams to ensure a seamless and compliant care experience.

Hybrid: Full-time hybrid position offering autonomy while utilizing your assessment, care planning and case management skills.

Conduct in-home client assessments and evaluate needs against nursing standards

Develop individualized plans of care following each home visit, reflecting client preferences and physician direction

Obtain and maintain signed physician orders; Provide written instruction and hands-on training so personal care workers demonstrate skill mastery before working independently

Support new caregivers through orientation and offer ongoing mentorship in the field

Conduct scheduled and unannounced supervisory home visits at least every 60 days — observing care delivery, auditing daily records, and documenting any changes needed

Serve on the Quality Assessment and Assurance Committee and contribute to operational planning

Active, unrestricted Wisconsin RN license (or multistate compact privilege)

Experience in home care, personal care, or community-based nursing preferred
