We are seeking a dedicated and compassionate Social Worker to join our team, a 235-bed facility located in Berkeley Township, New Jersey. As a Social Worker at our facility, you will play a vital role in providing long-term skilled nursing care and short-term post-acute rehabilitation to our patients.
As a Social Worker, you will work closely with our interdisciplinary team to provide high-quality care and support to our patients. Conducting assessments and developing care plans to meet the psychosocial needs of our patients
Provide psychosocial assessments and develop care plans to meet the needs of our patients
Bachelor's or Master's degree in Social Work (MSW) from an accredited program
Minimum of 2 years of experience in a healthcare setting, preferably in a social work role
Strong knowledge of social work principles, theories, and practices
A dynamic and supportive work environment that fosters growth and development
