We are currently seeking a compassionate and experienced PRN Registered Nurse (RN) to join our Boca Raton team.If you enjoy working independently, building meaningful relationships with patients, and making a difference in the community, we'd love to hear from you!Position SummaryThe PRN Registered Nurse is responsible for providing skilled nursing care to patients in their homes in accordance with physician orders, agency policies, and Medicare regulations. ResponsibilitiesPerform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits.Develop, implement, and update individualized plans of care.Administer medications, treatments, wound care, injections, and other skilled nursing services.Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team.Collaborate with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides to coordinate quality patient care.Maintain compliance with Medicare Conditions of Participation, state and federal regulations, and agency policies.Travel throughout the assigned coverage area to provide patient care.Current, active Florida Registered Nurse (RN) license.Minimum of one (1) year of nursing experience required.Home Health and Medicare experience preferred.Valid Florida driver's license, reliable transportation, and current auto insurance.Ability to work independently and manage a flexible schedule.Why Senior Care Solutions?At Senior Care Solutions, our mission is to provide compassionate, high-quality care that allows patients to safely remain in the comfort of their homes.
