Weekend Matron

Overview Market leading healthcare organization looking for a Psychiatric Mental Health Nurse Practitioner to join our team as a member of the Interdisciplinary Team. This is a full‑time Monday‑Friday 8‑4, no nights, weekends or holidays position covering Lynn and Brighton sites. The role includes behavioral health services for complex older adults in assisted living and skilled nursing facilities.
Responsibilities Perform comprehensive initial behavioral health evaluations for complex older adults in various settings.
Evaluate participants for acute changes to mitigate risk of psychiatric hospitalization and provide close follow‑up.
Make nursing home visits in accordance with state requirements.
Make home visits to participants as needed.
Follow up with participants routinely or urgently as clinical need arises.
Develop appropriate behavioral health treatment plans and monitor medication regimens.
Implement gradual dose reductions (GDR) as needed within regulatory practices.
Monitor clients' response to psychotropic medications and adjust treatment plans as warranted.
Document evidence‑based interventions with emphasis on psychopharmacology management.
Participate in behavioral health initiatives to improve clinical practice, teamwork, and program development.
Cover for Behavioral Health colleagues when needed.
Provide behavioral health teaching to participants, families, caregivers, and the Interdisciplinary Team.
Adhere to the standards in the NP/MD Collaborative Practice Guide.
Complete accurate and timely documentation.
Ensure fiscal responsibility within budgetary constraints.
Maintain the highest standards of care in all interactions with participants.
Pass an N95 mask fit test and wear mask where the seal is critical.
Travel locally as required.
Perform other duties as required.
Qualifications & Skills Master of Science in Nursing (MSN).
Current Board Certification as a Psychiatric Mental Health Nurse Practitioner.
Current licensure as a Nurse Practitioner in Massachusetts.
Current Massachusetts Controlled Substances Registration and DEA Prescription Privileges, CPR Certification.
At least five (5) years of experience working with a geriatric population.
Preferred at least one (1) year of experience with low‑income and diverse cultural groups.
Preferred prior skilled nursing facility experience.
Ability to obtain and maintain contracted facility credentials and privileges.
Demonstrated ability to work within urban communities with emphasis on racially, ethnically, culturally diverse, low‑income, and special‑needs populations.
Strong teamwork, respectful communication, and problem‑identification skills.
COVID‑vaccinated preferred.
