About This Role The Clinical Care Coordinator, RN in Home Care is responsible for providing telephonic case management and ensuring quality patient care. This role is ideal for experienced RNs who excel in collaboration and patient advocacy.

What You'll Do Provide telephonic case management and quality initiatives.
Collaborate with specialists and healthcare professionals for home health services.
Utilize the nursing process to manage a nursing case manager caseload.
Provide comprehensive care and teaching to patients and families.
Assist client care coordinators in prioritizing patient visits.
Collaborate with community agencies to identify patient resources.
Document assessment data and patient care activities.
Requirements Education Associate's Degree in Nursing or Diploma Certificate in Nursing
Experience Minimum of 2 years RN experience
Minimum of 3 years certified home care experience
Licenses & Certifications NJ State Professional Registered Nurse License
BLS AHA Basic Health Care Life Support HCP Certification
