We are seeking a dedicated and experienced Licensed Practical Nurse to join our team at Warrensburg Manor Care Center, a unique skilled nursing and rehabilitation facility that prioritizes the comfort and well-being of our residents. As a key member of our healthcare team, you will be responsible for ensuring the highest quality of care for our patients in a calm and comfortable environment.

Responsibilities

Review and analyze medical records to ensure accurate and complete documentation

Collaborate with healthcare providers to obtain necessary information for proper coding and billing

Conduct assessments of patient's medical conditions and develop care plans

Coordinate and facilitate communication between healthcare team members, patients, and families

Ensure compliance with Medicare regulations and guidelines

Perform utilization management activities to optimize patient care and resource allocation

Maintain accurate and up-to-date records of patient information and treatment plans

Requirements

Previous experience working in a medical office or healthcare setting

Strong knowledge of patient care practices and documentation review processes

Familiarity with primary care, Medicare regulations, and coding systems (ICD-9)

Experience in pediatrics or nursing home settings is a plus

Excellent organizational skills with attention to detail

Ability to effectively communicate with healthcare professionals, patients, and families

Proficient in utilizing electronic health record systems for documentation purposes

On call rotation
